Preparing for Your NDIS Plan Review in Ballarat

An NDIS plan review is an opportunity to look at how the past period has gone and talk about the support that may suit you next.
Preparing ahead of time can make the conversation clearer and less stressful. You do not need to have everything worked out. The aim is simply to explain your situation, your goals and the support that helps you day to day.
Start by reflecting on the past year
Before the review, it can help to think about what has worked well and what has been harder than expected. Notes made over time are often more accurate than trying to remember everything in one sitting.
- Which supports you used most often
- Supports that were funded but rarely used
- Goals you worked towards and any progress made
- Changes in your health, routine, living situation or support network
- Activities you would like to do more of
Gather useful information
Having relevant documents together can make it easier to explain your circumstances. What is appropriate will depend on your situation and any requirements that apply to your plan.
- Your current NDIS plan
- Reports or letters from relevant professionals, where available
- Notes about your daily routine and where support is needed
- A list of the providers you currently work with
- Any equipment, access or transport needs
Think about your goals
Goals guide how a plan is written, so it helps to describe them in everyday language. A goal can be as practical as travelling independently to a regular activity, or as broad as building confidence in the community.
If your goals have changed, say so. Plans are intended to reflect the person you are now, not the person you were when the last plan was written.
Involve the people who support you
Family members, carers, support coordinators and support workers often notice practical details that are easy to overlook. With your consent, they can help describe what a typical week looks like and where extra support makes a difference.
You remain at the centre of the conversation. Other people can add information, but the decisions and preferences are yours.

- What does each funded support in my plan cover?
- How is my funding managed and what does that mean for provider choice?
- What happens if my needs change during the plan period?
- Who can help me coordinate services?
- What should I do if a support is not working for me?
